General Requirements
Application Form

Birth Certificate
Original PSA-certified Birth Certificate (with one clear photocopy).
Report Card
Original SF9 or Report Card from the last school year attended.
Good Moral
Certificate of Good Moral Character signed by the Principal or Counselor.
